CORONAVIRUS UPDATE:

Following Government guidelines ALL patients visiting the practice will be asked to wear a face covering. If you present at the practice without a face covering we will have to send you away .

Face coverings are not only to protect yourself but also the safety of other patients as well as our Doctors, Nurses & other staff members.

We thought this handy government link on ways you can make your own face coverings would be helpful; they don’t have to be a surgical masks.

IF ANYONE WHO LIVES IN YOUR HOUSEHOLD DISPLAYS ANY OF THESE SYMPTOMS THEN YOU MUST ISOLATE FOR 14 DAYS.

PLEASE CONTACT YOUR GP ONLY IF YOU HAVE SHORTNESS OF BREATH OR CHEST PAIN .

If you have serious symptoms that you cannot manage at home, please use NHS 111 online (Please only call NHS 111 if you cannot get online).

After 7 days of self-isolation, if you feel better and no longer have a high temperature you can return to your normal routine. If you have not had any signs of improvement after 7 days and have not already sought medical advice, you should use NHS111 online (you should only call NHS111 if you cannot get online) before you leave your home or let visitors in.

Cough may persist for several weeks in some people, despite the coronavirus infection having cleared. A persistent cough alone does not mean that you must continue to stay at home for more than 7 days.

Extended Hours

Patient Access: When you request to book appointments online a print off with your log in details is generated and left for collection at the front desk. If you haven't received your log in details yet, why not ask the receptionist when you next visit the surgery!

LETTERS OF MEDICAL EVIDENCE/MITIGATING CIRCUMSTANCES

These can only be provided when there is a medical record of the reason why the extension or mitigating circumstance is being claimed. Records cannot be made retrospectively. Please book an appointment with Sister Sarah Clifford in the first instance. For further information refer to the sickness absence policy on Ibis Intranet. There is a £20 charge for these letters, and you will need to pay before the letter is dictated.

CAMP AMERICA FORMS

These can be completed for you, often without an appointment. Please bring forms to reception as soon as you receive them.  If an appointment is required you will be contacted regarding this. The fee without an examination is £30 and with an examination is £40

MEDICAL REPORTS AND EXAMINATIONS

The doctors will carry out medical reports and examinations for employment or University purposes. To arrange an appointment, please contact reception.

General forms (without report) including: PGCE, British Council, Pre-Employment Forms, Medical Evidence letters for DSA & Declaration of Health forms are all £30
