Appointments for NEW problems

As of 3 June 2024, to request an appointment for a new problem you will need to complete a PATCHS online consultation form by CLICKING HERE.  Once a form has been completed, this will be triaged by a GP who will then advise on how to proceed (this may be an appointment with a GP or another healthcare professional).

Patients that do not have access to the internet are able to ring us on 01244 346677 and a PATCHS form will be filled in on their behalf.

Your Appointment

You may be offered a consultation:

  • By telephone
  • Face to face at the practice
  • At home (for housebound patients only) please telephone before 10:00 to request a visit at home.

Follow-up Appointments

When a more in-depth consultation is required, a follow-up appointment will be recommended.

Follow-up appointments can be booked up to 4 weeks in advance.

Please telephone after 10:30am and advise the receptionist that you wish to book a follow-up appointment.

Evening & Weekend Appointments

The Enhanced Access Appointment Service is run by Primary Care Cheshire.  Patients can book a routine appointment outside of our usual opening hours.

Clinics are available Monday to Friday 6.30pm to 8.30pm and Saturday 9am to 5pm. The clinics are held at Garden Lane Medical Centre, please ask Reception for details. You will not be offered an appointment with your own GP, but you will be seen by a suitably qualified health professional, such as a doctor or practice nurse.

The Reception team at Garden Lane are now able to book appointments at Enhanced Access on your behalf. Please contact the surgery as you would for a practice appointment.

CANCELLING AN EVENING OR WEEKEND APPOINTMENT

CONSENT & TREATMENT

 

  • Garden Lane Medical Centre aims to offer high quality care and treatment for all patients.
  • When this treatment is carried out by clinicians of the practice, all patients will receive full information regarding the procedure.
  • The clinician will seek consent from the patient and this will be recorded in the medical record.
