Confidential information from your medical records can be used by the NHS to improve the services offered so we can provide the best possible care for everyone.
This information along with your postcode, NHS number, date of birth and gender (but NOT your name), is sent to a secure system where it can be linked with other health information.
This allows those planning NHS services or carrying out medical research to use information from different parts of the NHS in a way which does not identify you.
The type of information shared, and how it is shared, is controlled by law and strict confidentiality rules.
YOU HAVE A CHOICE: If you are happy for your information to be used in this way you do not have to do anything. PLEASE NOTE: We have been advised that we are unable to AUTOMATICALLY opt you out of this scheme (known as 'care.data') even if you have already opted-out of the NHS Summary Care Record sharing scheme and/or the Local Care Record sharing scheme, so you will still need to complete an opt-out form specifically for Care.Data.
